Updated Waivers MUST be filled out for ALL programming beginning July 1, 2023. Follow the link below to fill out your waiver.

Admin staff will be checking the waivers before anyone steps out onto the gym floor so please be diligent when filling them out.

Misuse of these forms is grounds for CCGC staff to deny entry into our facility.
